Population Health Management is about identifying a population that is at risk for a particular disease and subsequently managing the risk of that subset. Since we know that 20% of an employer’s workforce consumes 80% of the health care dollar, it makes sense to manage those who are driving up the cost.

Traditional disease management programs focus largely upon the population that is already sick. They focus on the “train wrecks.” Yet year after year, a new batch of “train wrecks” emerge. The healthcare system is conditioned at throwing tons of resources at these few individuals in an effort to restore their health.
